DESCRIPTION OF REQUIREMENT The Oral Health Module of the Canadian Health Measures Survey (CHMS) is designed to collect information needed to assess the oral health status of Canadians and to assist in developing policy for oral care services. This Request for Proposal (RFP) is being issued for Statistics Canada to require the services of engaging registered dentists to fulfill the collection needs of the CHMS. QUALIFICATION Any interested bidder can compete, without having to confirm with the Contracting Authority, unless it would not be consistent with the business requirements of efficient operations. For example, the bidder cannot guarantee to complete and provide a COVID-19 Vaccination Requirement Certification. Should a bidder wish to compete, it may contact the Contracting Authority to request the RFP at any time prior to five (5) working days before the published bid closing date. In no circumstance will such an request require Statistics Canada to extend a bid closing date. EVALUATION PROCESS AND SELECTION METHODOLOGY Competition: Highest Combined Rating of Technical Merit 70% and Price 30% SECURITY REQUIREMENTS Contractor: reliability status (upon contract award) Personnel: reliability status (upon contract award) LOCATION OF WORK AND TRAVEL The training to the resources will be held in Ottawa, ON, and the exact address will be provided at least 6 weeks in advance. The work sites for the services will be the Mobile Examination Centres (MECs) across Canada (see Site Schedule in the RFP). The exact addresses of the MECs are to be determined on a per site basis and provided 6 weeks in advance. DURATION OF CONTRACT It is intended to result in the award of one (1) contract with the contract period from contract award to June 14, 2024.
