Description:
The Department of Veterans Affairs, Southeast Louisiana VA Health Care System (SLVHCS) intends to solicit proposals from contractors, to provide Community Based Outpatient Care to veterans and eligible beneficiaries being serviced in Washington Parish, Bogalusa, LA. The VA anticipates issuing a solicitation for this requirement on or about November 1, 2021 with a closing date for proposals on or about November 30, 2021. The solicitation will be available for download from https://sam.gov/content/opportunities. Interested contractors will be responsible for downloading copies of the solicitation from that website. No hard copies will be made available. Offerors are encouraged and are responsible for routinely checking this website for any solicitation amendments that may be issued throughout the proposal period. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the contract opportunities website, so they are notified of the issuance of the solicitation and any amendments. The contractor shall provide, at the contractor's facility, all necessary personnel, supervision, facilities, space, equipment, and supplies to provide community-based outpatient care as listed in the Performance Work Statement. The purpose of the solicitation will be to obtain proposals from contractors who can provide community-based outpatient care for veterans and eligible beneficiaries receiving care in and around the Washington Parish, Bogalusa, LA geographic area. The resulting contract will have a base term of one year, with options for nine (9) additional one-year terms exercisable at the discretion of the government. The Government anticipates using a tradeoff source selection method and will make an award of an indefinite delivery/indefinite quantity contract with fixed Per Member/Per Month rates for this requirement. North American Industry Classification System (NAICS) Code is 621498; size standard is $22.0 Million and the solicitation will be issued as unrestricted. All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in the SAM (System for Award Management) database (information on registration requirements can be obtained at SAM.gov | Entity Registrations. Questions must be submitted in writing via email to the Contracting Officer at toni.clendenen@va.gov. Questions must be received a minimum of ten (10) business days prior to the closing date listed in the RFP. If warranted, responses to inquiries will be incorporated into a written amendment.
