Combined Synopsis / Solicitation Posting: W81K0221Q0258 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for quotes are being requested and a written solicitation will not be issued. This solicitation W81K0221Q0258 of Purchase Request BPAMAMC2021-2 is issued as a Request for Quote (RFQ). Regional Health Contracting Office-Pacific, Joint Base Lewis-McChord (JBLM), Health Contracting Branch Lacey WA; intends to award multiple Blanket Purchase Agreements (BPAs) for the following source products: Source (1): FDA – Approved various consumable supplies for the Constellation and Infinity Vision Systems, to include vision contact implants and related instruments and supplies. This will be prepared in accordance with FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures as supplemented with additional information included in this notice. Joint Base Lewis-McChord, Health Contracting Branch (JBLM-HCB) is requesting product catalog and pricing as part of a Blanket Purchase Agreement (BPA) for the above listed FDA Approved Products to be used at the Medical Treatment Facilities (MTF’s) indicated in this synopsis; from interested, FDA approved Vendors, both Large and Small Businesses. See the attached Blanket Purchase Agreement Scope of Work and contact JBLM-HCB via email (see below), if interested. ALL PRODUCTS MUST BE FDA APPROVED, AND MEET THE SURGEON/PATIENT REQUIREMENTS UPON SELECTION. If vendors are other than the Original Manufacturers (OM), please provide evidence that there is an existing and ongoing business relationship that will allow distribution of the OM products. The Primary NAICS Code is 339113; FSC Code is 6515 These products are to support the following Medical Treatment Facilities: Madigan Army Medical Center, (MAMC), Joint Base Lewis-McChord, Washington Basset Army Community Hospital, (BACH), FT Wainwright Alaska. Tripler Army Medical Center, Ft. Shafter, Hawaii The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-7. Simplified Acquisition Procedures will be followed. Interested parties must be registered in the System for Award Management (SAM), online at www.sam.gov.
