This solicitation is a request for Non-Personal Service Contract for a Contractor to provide Operating Room Assistant (Supply Technician and Scrub Technician) for Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 - Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance. Services shall be performed in accordance with the attached Performance Work Statement (PWS) for both Supply Technician and Scrub Technician. This solicitation is Unrestricted for a Non-Personal Service Contract per FAR 37.401(a) for Operating Room Assistants services to be performed. The Period of Performance will be issued 6 months each 1) Base Period 2) Option Period 1 to extend the contract. Base Period: Date of Award through 6 months Option 1 Period: extend additional 6 months The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the operating room assistant will be released from contract assignment. IDENTIFY EACH PROFILE WITH THE DEPARTMENT APPLYING FOR: Two Positions (2) for Operating Room Assistant – Medical Supply Technician One Position (1) for Operating Room Assistant – Surgical Scrub (REQUIRED CALL BACK) **It is the responsibility of the Offeror to submit only “qualified”, applicable, and available personnel. Any personnel submitted by multiple Offerors will not be forwarded for evaluation and further consideration.** The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated. HOUSING: No government housing available for NNMC. The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns REQUIREMENTS: To be considered for award, your offer shall include the following documents: 1. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information- phone number/email address, Professional Operating Room Assistant Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 7.0 See attached Requirements for Operating Room Assistant for Surgical Scrub and Supply Technician Requirements 4. Availability of candidate(s). 5. The Contractor will start until the Purchase Order is signed by Contracting Officer 6. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value.
