2 SCOPE OF WORK Dental Unit Waterlines, Biofilm, and Water Quality VA Sierra Nevada Health Care System, Reno, Nevada PART 1 - GENERAL BACKGROUND: The Centers for Disease Control and Prevention (CDC) and the American Dental Association (ADA) have recommended dental unit water line (DUWL) deliver should meet the U.S. potable water standard The VA Sierra Nevada Healthcare System (VASNHCS) Dental Clinic is located at 3674 S. Virginia St., Reno, Nevada, 89502. Statement of Work The staff member performing the work must be an American Conference of Governmental Industrial Hygienist (ACGIH) Certified Industrial Hygienist (CIH) or worker under the supervision of a ACGIH, CIH. All staff members involved in the physical sampling must be full time staff member of the company awarded the contract. The Contractor shall be familiar with CDC, Morbidity and Mortality Weekly Report, December 19, 2003/Vol. 52/No. RR-17, Guidelines for Infection Control in Dental Health Care Settings-2003. In addition, the Contractor will evaluate annually the ADA recommendations and provide recommended changes to the Chief of Dental and the Facility Industrial Hygienist. All services provided hereunder shall be performed in accordance with industry recognized, professionally published, internally developed, and/or client stipulated testing procedures. Sampling: The Contractor will perform one water sample per dental chair waterline. Water samples will be tested for Heterotrophic Bacteria. There will be a total of 24 samples. Samples will be collected and submitted to an ACGIH accredited laboratory in accordance with Standard Method 9215B Pour Plate using R2A agar. A sterile, disposable 120 ml plastic bottle containing sodium thiosulfate will be used to capture first catch samples. Sample containers must be received in the laboratory within 24 hours of sampling (or less if called for by the laboratory). Samples may be stored in refrigerated conditions temporarily prior to shipping. Samples will be shipped using a cooler and ice packs shall be used during sampling in the field to store containers until shipped. Any variance in sampling methodology must receive approval from the VASNHCS Chief of Dental and Facility Industrial Hygienist. Supplies: Contractor will provide all sampling media, sampling equipment, vehicles, shipping labels, chain of custody, shipping equipment (e.g. ice packs, coolers, etc..) and laboratory costs for proper analysis. Schedule: Contractor will sample once a quarter unless quarterly sampling results indicate a level greater than or equal to 500 Colony Forming Units (CFU). If 500 CFU results occur, the Contractor will sample once per waterline that indicated a positive, or more if necessary, until levels are dropped below 500 CFU. Positive results have occurred only once in the last three years. The facility does not expect to have more than 12 additional samples for any fiscal year. Sample Results/Analysis: Contractor will provide a copy of the analysis results through email to Chief of Dental, Chief of Safety, Pharmacy Hazardous Drug Coordinator, Chief of Operations & Maintenance, Infection Preventionist, Facility Industrial Hygienist, and Associate Director within 24 hours of performing analysis. Emails will be provided within 15 days of contract being awarded. A single hard copy will be sent to the Chief of Dental and Chief of Operations and Maintenance within one week of performing analysis. If the laboratory analysis indicates positive results greater than or equal to 500 CFU in any samples provided by the VASNHCS, a phone call will be placed immediately to the Contracting Officer Representative (COR) from VASNHCS and Chief of Dental. Quality Control 1) The contractor shall specify personnel that will serve as a single point of contact for all Quality Control issues. This person must be approved by the COR. 1.3 PERIOD OF PERFORMANCE This contract consists of one (1) base year and four (4) one-year contract option periods to coincide with the beginning/end of each fiscal year. Fiscal Year begins on October 1 and ends on September 30 respectfully.
