This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR (Federal Acquisition Regulations) Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 75H70622R00003. The solicitation document incorporated provisions and clauses are those in effect through FAC (Federal Acquisition Circular) 2022-01. This is a Total Small Business Set-aside solicitation. NAICS code is 812320 – Drycleaning and Laundry Services (except Coin-Operated), Small Business Size Standard is $6.0 Million; Product Service Code is S209 – Housekeeping – Laundry/Drycleaning. The unit pricing must be all inclusive (to include but not be limited to travel, lodging, per diem, fringe benefits, federal, state, and local taxes) plus all other costs pertinent to the performance of this contract. Laundry Services for 145,000 LBS @$__________ /LB, totaling $__________ Option Year Four (4): January 1, 2026 through December 31, 2026 Laundry Services for 145,000 LBS @$__________ /LB, totaling $__________ Total $_______________ The Great Plains Area Office is soliciting proposals for a Firm Fixed-Price, Indefinite Delivery/Requirements, Non-Personal Services contract for Laundry Services for the Rosebud IHS Hospital, 400 Soldier Creek Road, Rosebud, SD 57570. Laundry Services include: (List is not all-inclusive. Refer to the Statement of Work.) 1. Collection and delivery of various types of laundry three times a week (Monday, Wednesday, and Friday). 2. Rough dry, flat work, finished work, pillows, mops, and dry cleaning of drapes. 3. Laundering to be done in accordance with JACHO and CMS acceptable standards. The period of performance will be from January 1, 2022 through December 31, 2022, with four 1-year options with performance dates of January 1, 2023 through December 31, 2023; January 1, 2024 through December 31, 2024; January 1, 2025 through December 31, 2025; and January 1, 2026; through December 31, 2026; respectively. INSTRUCTION TO OFFERORS: Proposal must contain the following documents in order to be considered responsive and eligible for an award: 1. For each applicable employee, a completed and signed Agreement to a Temporary Waiver of Character Investigation form. 2. For each applicable employee, a completed and signed Optional Form 306 Declaration for Federal Employment. 3. For each applicable, a completed and signed Optional Form 306 Addendum to Declaration for Federal Employment. Completion of the attached FAR and HHSAR (Health and Human Services Acquisition Regulations) provisions and clauses applicable to this acquisition, including: FAR 52.212-1 - Instructions to Offerors – Commercial Items (Nov 2021). This information must be submitted by the Offeror. See Section L for full text. FAR 52.212-2 – Evaluation – Commercial Items (Nov 2021). Basis for Award is LPTA (Lowest Price, Technically Acceptable). The Government will award a contract resulting from this solicitation on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. The following factors shall be used to evaluate offers: Ability to meet or exceed the Performance Objectives, Performance Measure, and Performance Standards listed in the matrix. Price Evaluation – The Government shall conduct a price evaluation of all technically acceptable offers. Award will be made on an “all-or-none” basis to the lowest responsive and responsible offeror. FAR 52.212-3 – Offeror Representations and Certifications – Commercial Items (Nov 2021). This must be completed by the Offeror. See attachment for full text. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (q) of this provision.
